TRANSPORTATION SERVICE INFORMATION

In recent times, we have been inundated with questions regarding our transportation service. Please go over the following and if you would like to be sent a "Registration Medical Form", please call us. Additionally, we would like to take this opportunity to thank the many passengers who use our service correctly, such as ready at the set time, no cancellations, and patience when waiting for their return ride.

We ask that you carefully read and follow these rules, as we serve many Hazel Park residents.

Service is available to Hazel Park residents, age 55 or older, retired or semi-retired, with no other means of transportation and to the handicapped residents.

The fee for transportation is $1 per trip per person ($2 per round trip) with the exception of lunch pick-ups.

Requests for rides must be made Monday - Friday, 9:00a.m. to 1:30p.m. Call no less than two days in advance.

When calling for a ride, you "MUST" provide the address & phone number to your destination otherwise your appointment may not be taken.

The van hours of operation are 8:30 a.m. to approximately 3:00 p.m., Monday - Friday.

This is a "curb to curb" service. Drivers may help you in and out of the van.

Be ready at least 15 minutes before your pick up time.

Wheelchair transportation is available. Rides are on a first come, first serve basis.
Please note: Motorized wheelchairs are difficult for the lift to handle; we request your cooperation in using a manual wheelchair whenever possible.

Medical appointments receive priority. All appointments must be before 1:00 p.m., allowing the last pick up by 3:00 p.m.

Transportation service will be limited to a 7 mile radius of Hazel Park.

All riders will be picked up from their Hazel Park residence for their appointments.

All banks, beauty shops, barbershops, and grocery stores must be located in the Hazel Park area. Grocery store trips out of the city should have minimum 4 passengers to schedule a van. Grocery bags should be limited to 3 bags.

You must notify the office if you are a one-way trip. If you get an alternate ride home, you must notify the office.

Be aware: YOU MAY HAVE A WAIT TO BE PICKED UP! It seldom happens, but your wait may be as long as 1 Þ hours. Please bring a book or something to occupy your time while waiting.

Mall trips are scheduled by the office. No individual Mall trips accepted.

Call the office as soon as possible if you must cancel. We may be able to give your time to another passenger.

There is a probationary clause in our rules, as S.M.A.R.T. has recommended. These are some of the reasons you may be placed on probation:

If you cancel more than 2 appointments in a row, you will be placed on probation for 30 days. If the problem continues you may be denied use of the transportation program.

No passengers are allowed 2 rides or drop offs in one day, out of Hazel Park, as this causes confusion & runs the drivers behind.(e.g. Dr. & druggist is allowed in Hazel Park)

DO NOT LEAVE your original destination; the driver will not pick you up from somewhere else, because this also adds confusion to the program.
Drivers are there to help you but they have rules they must follow also. Your consideration & understanding of their schedule is appreciated but note: Verbal Abuse will not be tolerated.

We appreciate your cooperation in the aforementioned rules/guidelines, which will help us to offer this most valuable service to the Citizens of Hazel Park.
