Dog License
Please bring the following information to the City Clerk's department to obtain a dog license:
The original rabies certificate
A check/cash made out to the City of Hazel Park for the cost of the license
A daytime phone number in case we have any questions
The rabies certificate will be returned along with the receipt and the dog tag.
Price List
Cost of a dog license is determined by the length of time the rabies shot is valid.
1-Year Rabies Vaccination $8.00
2-Year Rabies Vaccination $12.00
3-Year Rabies Vaccination $16.00
Important Note: If your dog’s rabies vaccination is for 1 year, then you are only eligible to receive a 1-year license. The same is true if your dog received a rabies vaccination for 3 years, then you would only be eligible for a
3-year license.
